Abstract:Recently,Ohsawa et al. provide evidence that inhaled hydrogen gas has antioxidant and antiapoptotic activities that protect the brain and liver against ischemia-reperfusion injury. In fact,there is some endogenous hydrogen produced by intestinal bacteria within animal and human.The concentration of hydrogen in some mice tissues reached the antioxidant effect in their paper demonstrates,so we think that hydrogen should be an endogenous antioxidant in the body.

Abstract:Objective:To prepare 2 composites using different proportions of hydroxyapatite and collagen and to assess their structural and biological properties, so as to pave a way for preparing tissue engineering chest wall scaffold.Methods: Two kinds of hydroxyapatite/collagen composites were prepared according to the weight ratios of 11 and 12; collagen sponge served as control. Then the structures of the 2 composites and the collagen sponge were observed under SEM. In vivo study was conducted to assess the biocompatibility and biodegradation of the composites by gross inspection and histological examination. Results: The collagen sponge had a 3-D network structure with fluey collagen fibers and poor mechanical strength, and its structure was damaged within 2 weeks after implantation and was completely absorbed 4 weeks later. The hydroxyapatite and collagen were well mixed in the composite with a hydroxyapatite to collagen ratio of 12; the composite had homogeneous 3-D porous structure (size of the pore being 100-400 m) and showed good biocompatibility: maintained its porous structure 4 weeks after implantation and was absorbed within 8 weeks. In composite with hydroxyapatite to collagen ratio of 11, the hydroxyapatite particles were separated from collagen fiber and conglomerated into masses, and the composite resulted in severe tissue reaction after implantation.Conclusion: When mixed with a reasonable proportion of hydroxyapatite, the collagen sponge has improved structure, biodegradable performance, and biocompatibility; the composite may be a novel scaffold for tissue engineering chest wall reconstruction.

Abstract:1临床资料患者男性,46岁。因双下肢出现“紫癜”、水肿、蛋白尿26 d于2003年2月21日入院。9年前曾有“乙肝”史,未治“自愈”。入院时皮疹已完全消退。查体除双下肢水肿外无明显异常发现。尿常规:蛋白5.00 g/L,RBC 150/μl(15~20/HP);WBC 25/μl(5~8/HP);24 h尿蛋白定量7.92 g。Alb 25 g/L,Glo 27 g/L,ALT 20 IU/L,AST 14 IU/L;T-ch 7.78 mmol/L,TG 1.60 mmol/L。HBsAg(,HBsAb+,HBeAg-,HBeAb+,HBcAb+;PCR HBV DNA(-)。BUN 2.7 mmol/L,Scr 77 μmol/L,Glu 4.9 mmol/L。血常规:WBC 5.1×109/L,RBC 4.9×109/L,Hb 158 g/L,Plt 93×109/L; IgG 5.86 g/L,IgA 2.63 g/L,IgM 1.56 g/L,C3 1.31,C4 0.43。X线胸片、心电图均未见异常。B超:右肾10.7 cm×4.9 cm,左肾11.7 cm×5.5 cm。未见明显异常。肾穿刺活检结果:光镜下见肾小球毛细血管襻不规则增厚。系膜区呈局灶节段性分布的轻度至重度增宽伴系膜细胞轻度至中度增生;数个节段可见单个核、多形核细胞浸润伴明显球囊粘连及小新月体形成;血管襻系膜区可见团块状嗜复红蛋白。肾小管上皮细胞颗粒变性,刷状缘脱落,管腔轻度扩张,小片萎缩。间质散在及1处小片状炎细胞浸润。小动脉未见明显异常。免疫荧光:IgA()在系膜区沉积。IgG、 IgM、C3、C1q均阴性。免疫酶标:HBsAg-、HBcAg-、CoⅣ均阴性。电镜下肾小球基底膜未见增厚,毛细血管襻腔内可见多形核细胞浸润。上皮细胞胞质空泡变性。系膜区节段性轻度至中度增宽,细胞增生不明显。间质可见少量炎细胞浸润。病理诊断为:继发性IgA肾病,紫癜肾炎。[第一段]

Abstract:Objective:To investigate the relationship between p38 mitogen-activated protein kinase (p38MAPK), nuclear factor-κB (NF-κB) and cyclooxygenase-2 (COX-2), so as to study the roles of p38MAPK, NF-κB and COX-2 in diabetic nephropathy. Methods: Rat mesangial cell line HBZY-1 was incubated with certain concentrations of glucose(25 mmol/L), insulin(100 mmol/L), H2O2(100 mmol/L) and glycation endproducts (AGEs)(100 mg/L) with or without pretreatment with SB203580, a specific inhibitor of p38MAPK, then the expression of p38MAPK, NF-κB and COX-2 in the HBZY-1 cells was examined. Results: High glucose, high insulin, H2O2 and AGEs independently activated p38MAPK, increased p38MAPK phosphorylation, and significantly up-regulated the expression of NF-κB and COX-2 compared with the control group (P<0.01). Pretreatment with SB203580 significantly decreased expression of NF-κB and COX-2 compared with the corresponding stimulating group (P<0.01). Conclusion: p38MAPK may induce renal damage during diabetic nephropathy by activating NF-κB and COX-2, indicating that p38MAPK, NF-κB and COX-2 may play important roles in the development of diabetic nephropathy.

Abstract:Objective:To investigate the effect of repetitive transcranial magnetic stimulation(rTMS)on dopaminergic neurons and brain-derived neurotrophic factor(BDNF) in the substantia nigra (SN) of mouse with Parkinson’s disease (PD ), and to disclose the possible mechanisms. Methods: Thirty-two male C57BL/6J mice were equally randomized into normal saline (NS), sham-rTMS (s-rTMS), PD model and rTMS groups. PD model was established with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) in mice. The animals in rTMS group received 5 trains of 1 pulse/s for 25 s, at an intensity of 1 Tesla (T) daily for 2 weeks. After the treatment of rTMS, the changes in expression of tyrosinehy droxylase (TH) and BDNF in the SN of animals were observed by immunohistochemical technique; the quantitative analysis was performed by image analysis system. Results: Compared with NS group, the numbers of TH and BDNF immunoreactive(TH-ir and BDNF-ir)cells and the corrected optical density (CD) values in PD and s-rTMS group were significantly lower than those in the NS group (all P<0.01); the numbers in rTMS group were significantly higher compared with those in the PD and s-rTMS groups (all P<0.05); and the numbers were not significantly different between the s-rTMS and PD groups(P>0.05). Correlation analysis showed a positive correlation between the count of TH-ir and BDNF-ir cells (r=0.949, P<0.01); and positive correlation was also noted between the CD values of TH-ir and BDNF-ir cells (r=0.880, P<0.01).Conclusion: It is suggested that rTMS has protective effects on dopaminergic neurons in the SN of PD mice, and the mechanism might be related to the up-regulation of BDNF expression.

Abstract:Objective:To explore the protective effect of leptin against rats intestinal ischemia/reperfusion injury and the related mechanism. Methods: Ninety-nine rats were randomly divided into 11 groups (n=9),namely the sham-operation group,ischemia/reperfusion groups (45 min ischemia plus 15 reperfusion \[I45R15\],I45R30,I45R60,I45R180,I45R360)and the 5 corresponding leptin-treated ischemia-reperfusion groups. The serum levels of TNF-α and IL-10 were determined in all groups and the histopathological changes were observed in the small intestine. Results: Compared with the sham-operation group,the serum levels of TNF-α and IL-10 were significantly increased in the ischemia/reperfusion groups (both P<0.05),with obvious pathological damage observed in the ischemia/reperfusion groups. Leptin treatment significantly decreased TNF-α level,increased IL-10 level,and alleviated the damage of the small intestine (all P<0.05) of animals after ischemia /reperfusion injury.Conclusion: Leptin can alleviate the intestinal damage in rats after intestinal ischemia/reperfusion through down-regulating TNF-α level and up-regulating IL-10 levels.

Abstract:Objective:To construct a recombinant retroviral vector carrying human inducible costimulator (ICOS) gene,screen for CHO cell line stably expressing ICOS protein and to study its biological activity.Methods: ICOS cDNA was obtained from human peripheral blood mononuclear cells (PBMC) through RT-PCR and was cloned into retroviral vector to construct retroviral recombinant pMSCV-ICOS; the latter was then packed and the high-titer virus producing cells were screened.Then CHO cell was infected by this high-titer virus and the stable cell line was screened.CHO-ICOS cells were co-cultured with PBMC (the ratio of CHO-ICOS to PBMC being 11,12,15, and 110) in presence of substimulating dose of anti-human CD3 antibody.The proliferation of PBMC and the CD25 expression on T cells were examined by 3H-TdR incorporation method and flow cytometry,respectively.CHO-pMSCV cells co-cultured with PBMC (11) served as the negative control and PBMC served as blank control.Results: We successfully constructed the retroviral recombinant pMSCV-ICOS and obtained CHO cell line stably expressing ICOS protein.3H-TdR incorporation method and flow cytometry showed that,compared with the negative control group and the blank control group,co-culture with CHO-ICOS cells significantly inhibited the anti-CD3 antibody-induced activation and proliferation of PBMC(P<0.05);the maximal inhibitory rates of activation and proliferation were obtained when the ratio of CHO-ICOS to PBMC was 11,being (68±5.9)% and (44.08±3.26)%,respectively.Conclusion: CHO cell line stably expressing ICOS protein has been successfully established,which lays a foundation for future study.

Abstract:Objective:To study the therapeutic effect of 2 artificially synthesized fusion peptides on scalded mice. Methods: Two new fusion peptides, M-T508 and M-Tα1, were synthesized based on amino acid sequences selected according to the characteristics of scald (including 3 stages: inflammation, infection and repair by cell proliferation). The 2 peptides were applied for second degree scald burn in mice and the healing of the scald burn was observed. The expression ofⅠcollagen, vascular endothelial growth factor (VEGF), proliferating cell nuclear antigen (PCNA), and cytokeratin (CK) was detected by immunohistochemical staining. Serum TNF-α concentration was determined by ELISA in the animals and the blood vessel permeability was analyzed by Evans blue staining. Results: The healing periods of scald burns in the 2 peptide groups were 1 week earlier than that in the control group. Immunohistochemical results showed that the expression of collagenⅠ, VEGF, PCNA and CK was all positive in the 2 peptide groups 10 days after scald and negative in the control group. Examination of blood vessel permeability and serum TNF-α concentration in the M-Tα1 and M-T508 groups showed that the 2 fusion peptides had anti-inflammation and anti-infiltration effects in scalded mouse. Conclusion: The synthesized fusion peptide M-Tα1 and M-T508 can promote the healing of scald burns in mice.

Abstract:Objective:To investigate the expression of protein MUC1 and MUC2 in intraductal papillary mucinous neoplasms (IPMNs) and its significance.Methods: Immunohistochemical method (EnVision) was used to analyze the expression of protein MUC1 and MUC2 in 18 IPMNs and 9 pancreatic ductal adenocarcinomas.Results: Expression of protein MUC1 was detected in 4 of the 18 (22%) IPMNs (including 1 with pancreatic intraductal papillary-mucinous carcinoma) and all the 9 (100%) the pancreatic ductal adenocarcinomas,with the latter significantly higher than the former(P<0.01).Expression of protein MUC2 was detected in 17 of the 18 (94.4%) IPMNs and in 1 of the 9 (11.1%) pancreatic ductal adenocarcinomas,with the former significantly higher than the latter(P<0.01).The expression levels of MUC2 were significantly different in IPMNs of different types (IPMA,IPMB,and IPMC,P<0.05).Conclusion: IPMNs have high expression of MUC2 and the expression is associated with the pathological types of IPMN.MUC1 expression may serve as a marker of malignant IPMNs and is worth further studying.

Abstract:Abstact Objectives To explore the effects of the primary affection and relative risks on occurance and developing of contrast material nephropathy. Methods Retrospective analysis have been taken on 386 patients who were given with angiography before. Results Acute renal injuries have been observed in 22 patients among 386 cases. The incidence was 5.7%. Also we can find that the incidence was the highest in diabetes patients with about 17%. Conclusion Acute renal injuries induced by contrast medium mainly lie on whether the risks exist or not, which should be thought much.

Abstract:Objective: To study the relationship of TSP-1 expression with the metastasis and prognosis of human pancreatic cancer. Methods: The expression and distribution of TSP-1 were investigated in 74 pancreatic cancer tissues and 16 other pancreatic tissues (6 normal pancreatic tissues, 4 chronic pancreatitis tissues and 6 normal pancreatic tissues adjacent to the pancreatic cancer tissues) using immunohistochemical method. The data were analyzed by using SPSS 11.0 software and the value of TSP-1 expression in predicting the metastasis and prognosis of pancreatic cancer was investigated. Results: The expression of TSP-1 in the pancreatic cancer tissues was significantly higher than that in the normal pancreatic and chronic pancreatitis tissues (P<0.05). Significant differences were also found between tumors of different stages, different sizes, with or without lymphatic metastasis, and with or without neural infiltration (P<0.01 or P<0.05). Logistic regression analysis showed that TSP-1 expression was correlated with the stages and neural infiltration of the tumor. Univariate analysis using the Kaplan-Meier method and log-rank test revealed a relationship of TSP-1 expression with lymphatic metastasis, neural infiltration, tumor stage and patient survival rate. Multivariate analysis using the Cox proportional hazard model demonstrated that the neural filtration, lymphatic metastasis were the independent factors of patients’ prognosis and survival; TSP-1 expression was not independent prognostic parameter.Conclusion: Expression and distribution of TSP-1 may serve as indicator for metastasis of pancreatic cancer and is also helpful in predicting the prognosis of pancreatic cancer.

Abstract:Objective:To investigate the relationship of involvement area with the clinical,endoscopic and pathological characteristics of ulcerative colitis (UC). Methods: We retrospectively studied the clinical,endoscopic and pathological data of 75 patients with UC,including 17(22.67%) with distal colitis,27(36%) with left-sided colitis and 31(41.33%) with extensive colitis/pancolitis. Results: No differences were found in the gender and age of onset between different groups. The incidences of main clinical manifestations and complications increased with the increase of the UC involvement,with significant difference found in the incidences of anemia,hypoproteinemia and extraintestinal manifestations between different groups (P<0.05). Positive correlation was found between the UC involvement with the UC severity (r=0.418,P<0.001) and disease activity under endoscopy (r=0.239,P=0.039). The incidence of atypical hyperplasia of enteric epithelium in extensive colitis/pancolitis group was significantly higher than those in the other 2 groups (both P<0.05). Conclusion: The UC involvement is related to the clinical,endoscopic and pathological characteristics of UC; the involvement is positively correlated with the clinical severity and disease activity of UC under coloscopy.

Abstract:Objective:To discuss the diagnostic value of the new 5-sector approach for orbital imaging division in the diagnosis of orbital space-occupying lesions.Methods: We retrospectively analyzed the clinical and imaging data of 374 patients with orbital space-occupying lesions. The distribution of the lesions was analyzed according to the new 5-sector approach for orbital imaging division (globe compartment, optic nerve compartment, intraconal compartment, extraconal compartment and extra-periosteum compartment) and the distribution rule of the lesions were discussed. Results: Hemangioma was the most common lesion among our group, with an incidence of 34.75%(130/374); 79.23% (103/130) of the hemangiomas were distributed in the intraconal compartment. There was a distributional difference between the lymphoma and inflammatory pseudotumor(χ2=13.63, P=0.001),with the former mainly located in the extraconal compartment and the latter often in the mixed compartments. We also found that there were distributional differences for those lesions which had more than 10 sufferers in our study(χ2=788.058,P=0.001). Significant differences were also noted in the distribution of both benign and malignant lesions in the intraconal, extraconal and mixed type compartment(χ2intra/extra=66.53,P<0.001;χ2intra/mixed=88.17,P<0.001;χ2extra/mixed=5.078, P<0.05). For the benign lesions, the incidence order was: the intraconal compartment (99.18%,121/122)> extraconal compartment (56.12%,78/139)> mixed type(36.96%,17/46); for the malignant lesions, the incidence order was: mixed type(63.04%,29/46)> extraconal compartment(43.88%,61/139)> intraconal compartment(0.82%,1/122). Conclusion: There are distributional differences in orbital space-occupying lesions under the new 5-sector approach for orbital imaging division, indicating that the new 5-sector approach is helpful in the diagnosis of orbital lesions.

Abstract:Objective:To compare the therapeutic effects of penehyclidine hydrochloride on rat with oleic acid (OA)-induced acute lung injury (ALI) when given via inhalation and intravenous injection. Methods: Forty male Sprague-Dawley rats were randomly divided into 4 equal groups: normal control group(group Ⅰ),ALI group(group Ⅱ),penehyclidine hydrochloride inhalation group(group Ⅲ),and intravenous penehyclidine hydrochloride group(group Ⅳ). The blood pressure (BP),pulmonary artery pressure (PAP),and airway pressure (Paw) were determined every 60 min during the experiment and the artery blood gas analysis and mixed venous blood gas analysis were performed. The wet to dry lung weight ratio (W/D ) and the serum concentration of tumor necrosis factor alpha (TNF-α) were determined 240 min after the experiment in each group. Results: The OA-induced increase of PAP,Paw and decrease of PaO2/FiO2 were improved in the group Ⅲ and group Ⅳ compared with those in the group Ⅱ(P<0.05 for group Ⅲ). The W/D and serum TNF-α concentration were lower in the group Ⅲ and group Ⅳ compared with those in the group Ⅱ (P<0.05 for group Ⅲ). Conclusion: Penehyclidine hydrochloride can relieve the symptoms of OA-induced ALI. Inhalation approach is more effective than intravenous injection when the same dose of penehyclidine hydrochloride is given.

Abstract:Objective:To establish an animal model for anterior cervical-adjustable fusion fixator (AC-AFF),and to compare the operation time,blood loss,and recovery time of food intake and movement between AC-AFF group and other groups with different cervical reconstruction fixators.Methods: Eighteen goats were evenly randomized into 3 groups: AC-AFF,steel plate+titanium mesh,and steel plate+autogenous iliac bone group.The 3 fixtors were implanted after corpectomy.The operation time,blood loss during operation,and recovery time of food intake and movement were recorded in each animal and were compared.Results: All experiment animals survived after operation; one animal of steel plate+titanium mesh group suffered from incision infection and recovered 5 days after intramuscular penicillin therapy.Two animals in steel plate+autogenous iliac bone group could not stand on forehoof,but food intake was not influenced; the animals could stand 1 week later without treatment and walked with difficulty,and they recovered normal walk 2 weeks later.Four animals received C3 corpectomy (2 received AC-AFF and 2 received steel plate+titanium mesh) and the rest received C4 corpectomy.It was found that AC-AFF group had the shortest operation time (P<0.01),and there was no difference between the other 2 groups.The intra-operation blood loss in the steel plate+autogenous iliac bone group was significantly higher than that in the other 2 groups (P<0.01),with no significant difference found between the latter 2 groups.Conclusion: AC-AFF is easier to manage than the other 2 fixators; besides,AC-AFF has less blood loss,short operation time,less post-operation complication,and early recovery of food intake and movement.

Abstract:Objective:To investigate the inhibitory effect of adenovirus-mediated herpes simplex virus-thymidine kinase (TK) gene combined with α-IFN on renal clear-cell carcinoma. Methods: Adenovirus containing suicide gene TK, in combination with GCV or α-IFN, was used to treat human renal clear-cell carcinoma cell line 786-0, and the in vitro cytotoxic effects against 786-0 were evaluated using MTT method. The subcutaneous transplantation model of 786-0 cells was established with nude mice. Adenovirus containing TK gene was injected intratumorally and the GCV (50 mg/kg) was injected intraperitoneally; α-IFN (104 U/L) was injected intratumorally in combined therapy. The growth of tumors was observed after treatments. Results: The survival rate of 786-0 cells was (35.07±1.43)% in the TK+GCV+α-IFN group, (68.57±1.41)% in the TK+GCV group and (68.65±1.45)% in the α-IFN group ( P=0.000). There was an obvious synergic effect between Ad-TK and α-IFN in inhibiting 786-0 cells. Ad-TK combined with GCV and α-IFN significantly suppressed the growth of 786-0 cells growth in nude mice model.Conclusion: Adenovirus-mediated TK plus prodrug GCV combined with α-IFN has obvious therapeutic effect in treatment of human renal clear-cell carcinoma.

Abstract:Objective:To make a comparative study on the root and stalk of Actinidia valvata, so as to lay a ground for expanding medicinal part of Actinidia valvata. Methods: A comparative study was made between the root and stalk of Actinidia valvata on the following aspects: macroscopical properties, physical and chemical properties, contents of active constituents, and pharmacological actions. Results: The root and stalk of Actinidia valvata both showed positive reaction for alkaloids and saponin, and both had 280 nm absorption peak in UV-Vis spectrum. The aqueous extraction yield of the root and stalk were 5.35% and 5.68%, respectively; the contents of dihydrodehydrodiconiferyl alcohol were 0.005 01% and 0.006 41%, respectively. The IC50 of root and stalk extracts against HL60 were 62.39 mg and 75.51 mg, respectively; and against K562 were 70.47 mg and 77.46 mg, respectively. Their extracts had similar scavenging activity for free radicals and reductive effect; neither of them had analgesia actions.Conclusion: The root and stalk of Actinidia valvata have similar physical and chemical properties, contents of active constituents and pharmacological actions, suggesting that it is highly possible to substitute the root of Actinidia valvata with stalk for medicinal usage.

Abstract:Objective:To develop a HPLC method for the determination of tissue distribution of cinnarizine in rats. Methods: Cinnarizine (2 mg/Kg) was injected into the tail vein of rats. Tissue sample was alkalized with sodium hydroxide solution and extracted with methyl tert-butyl ether. The separation was performed on a Hypersil C18 column using methanol-water-glacial acetic acidacid-triethylamine (70300.60.4) as the mobile phase at UV 254 nm. The distribution of cinnarizine in the heart, liver, spleen, lung, kidney, stomach, small intestine, brain, fat and testes was determined. Results: The limit of detection of cinnarizine in the hepatic tissue was 0.02 μg /ml and the linear range was 0.05-1 0 μg/ml. The limit of detection in the intestine tissue was 0.02μg /ml and the linear range was 0.05-1 μg/ml, which meets the requirement for the analysis of biological samples. Cinnarizine was extensively distributed in rat body, with highest concentration found in the lung. Conclusion: Our method is easy to perform, accurate and sensitive, which makes it suitable for preclinical pharmacokinetic research of cinnarizine.

Abstract:Objective:To investigate the composition and clinical features of inpatients with congenital heart disease (CHD) complicated with pulmonary arterial hypertension (PAH) from a cardiovascular department.Methods: We retrospectively analyzed the clinical data of 238 PAH patients who were identified from 3 674 CHD inpatients in Changhai Hospital during Jan, 2001 to Jul, 2007.Results: Among the 3 674 CHD inpatients who were treated in our hospital in recent 6 years, 238 (6.48%) had PAH.PAH was found in 10.8% of patients with atrial septal defect (ASD), 2.81% of patients with ventricular septal defects (VSD), 5.57% of patients with patent ductus arteriosus (PDA), and 19.6% of patients with complex types.The mean age of patients was (45.6±17.7) years, with 49.6% of them being 40-60 years.Female patients accounted for 69.3% of the total.The composition of PAH patients was: ASD (62.6%), VSD(19.7%), and PDA(13.4%).The patients with VSD,PDA or other complex types were younger than those with ASD(P<0.01).More female PAH patients had ASD than VSD (76.5% vs 44.7%,P<0.01).Most patients had chest distress and dyspnea on exertion as initial symptoms and accentuated pulmonary component of the second heart sound as physical sign; they also had increased cardiothoracic ratio, prominent, dilated central pulmonary arteries on chest radiography, right ventricle enlargement, and incomplete right bundle branch block on ECG.The pulmonary artery systolic pressure(sPAP), as estimated by echocardiography, had a significant correlation with catheterization measurements(r=0.734,P<0.01).Conclusion: PAH is common in patients with CHD, especially in females, those with ASD, and those of 40-60 years old.The clinical presentation, chest radiography and ECG are normal.Echocardiography has important diagnosis value.There is a correlation between echocardiography and catheterization measurements of sPAP.

Abstract:Objective:To discuss the treatment for hemangiomas and vascular malformations. Methods: A retrospective study was conducted on 381 patients with hemangiomas and vascular malformations (January of 1998 to June of 2006). The patients consisted of 167 males and 214 females,with an age range from 1 year to 78 years (median age 16 years). According to the biological classification system introduced by Mulliken in 1982,98 patients had hemangiomas,38 had capillary malformations,203 had venous malformations,27 had arteriovenous malformations and 15 had lymphatic malformations. In all patients with hemangiomas,71 patients were treated with intralesional steroids,20 patients underwent surgical excision,7 patients were carefully monitored by doctors. In 38 patients with capillary malformations,27 underwent sclerosant,others were subjected to laser therapy or surgery. In 203 patients with venous malformation,136 received sclerotherapy,21 received surgical excision,and 11 underwent sclerotherapy conjunction plus surgical excision. Twenty-seven patients with arteriovenous malformations received sclerotherapy,interventional therapy,or sclerotherapy conjunction plus surgical excision. Fifteen patients with lymphatic malformations received sclerotherapy conjunction plus surgical excision. Results: All patients were followed-up for 0.5-6 years. Follow-up result showed that 199 patients were cured,125 were greatly improved,and 46 were partly improved; 10 patients showed no obvious improvement; and 1 patient died. Conclusion: Correct diagnosis and the classification,along with correct treatment strategy, are the keys to increase the treatment outcome and decrease complication of patients with hemangiomas and vascular malformations.

Abstract:Objective:To compare the influences of onychomycosis on quality of life between patients from Luoyang and Shanghai. Methods: A semi-structured questionnaire was used to analyze and compare the influences of onychomycosis on the quality of life of patients from Luoyang and Shanghai. Results: Ninety-six patients with onycomycosis were from Luoyang district, including 41(42.7%) males and 55 (57.3%) females. One hundred and eighteen patients were from Shanghai, including 52 (44.1%) males and 66 (55.9%) females. Onychomycosis had more severe influences on patients living in Luoyang than those living in Shanghai from the following 4 perspectives: subjective health feeling, emotional influence, thinking influence, and social communication difficulties. Onychomycosis had greater influence on patients who aged 41-50 years old, who had a disease course of 1-5 years, and who had a higher academic background. Conclusion:The influences of onychomycosis on quality of life are different for patients with different economic, cultural backgrounds.

Abstract:Objective:To identify a strain of bacterium (numbered ZJ8112) isolated from the sediments of the East China Sea and study its antibacterial activity.Methods: The ZJ8112 strain was subjected to phenotypical study,salt-aggregation test and 16S rDNA sequence analysis; the sequence analysis result was searched in the GenBank to determine its characteristics.The antibacterial activity of strain ZJ8112 was tested using the following as the indicator bacteria: Pyricularia oryzae,Bacillus subtilis,Escherichia coli,Saccharomyces cerevisiae,Candida albicans and Cladosporium cucumerinum.Results: We found that the ZJ8112 was a G+ bacterium and had central spore.Salt-tolerant study showed that the optimum salt concentration was 10% and it belonged to moderate halophile.Analysis of the 16S rDNA gene showed high similarity (99%) to the Bacillus subtilis F121112.The 16S rDNA sequence of strain ZJ8112 had been submitted to GenBank (accession number: EU100745).The cultural and phenotypical characteristics demonstrated that ZJ8112 is a strain of Bacillus.Bioactivity study showed that ZJ8112 had potent inhibitory activity on Escherichia coli and Pyricularia oryzae.Conclusion: ZJ8112 is a halophytic bacterium with an optimum salt concentration of 10%.ZJ8112 can strongly inhibit the Escherichia coli and Pyricularia oryzae,showing a potential role for antibiotics development.

Abstract:Objective:To synthesize optic pure L-norvaline. Methods: L-Norvaline was synthesized using n-butyraldehyde and potassium cyanide as starting material and (S)-(-)-α-methylbenzylamine as a chiral auxiliary agent by a selective Strecker reaction. Nearly optical pure α-amino nitrile was obtained by recrystallization; L-Norvaline was then obtained by hydrolysis and catalyzed debenzylation of α-amino nitrile. Results: The optimized reaction solvent system was chosen and high optical pure L-Norvaline was obtained by twice recrystallization procedure. Conclusion: Our method has less steps and is more selective than conventional methods and has a higher yield (47.4%).

Abstract:Objective:To set up a liquid chromatographic-tandem mass spectrometric (LC/MS/MS) method for determination of betamethasone in rabbit plasma.Methods: Betamethasone and the internal standard A0 were extracted from rabbit plasma by liquid-liquid extraction with diethyl ether-hexane (41,V/V). Chromatographic separation was performed on a Zorbax Eclipse XDB-C18 column with a mobile phase consisted of acetonitrile-5 mmol/L ammonium acetate-formic acid (80200.1,V/V/V) at a flow-rate of 0.60 ml/min. A tandem mass spectrometer equipped with electrospray ionization source was used as detector and operated in the positive ion mode. Quantification was performed using multiple reaction monitoring (MRM) of the transitions m/z 393m/z 373 and m/z 393m/z 355 for betamethasone,and m/z 489m/z 357 for the internal standard. Results: The linear calibration curves were obtained within the concentration range of 15.0- 1 000 pg/ml. The lower limit of quantification was 15.0 pg/ml. The intra- and inter-day relative standard deviation over the entire concentration range was less than 13.0%. The accuracy was in the range of -1.4% to -0.6% in terms of relative error. The method was applied to a pharmacokinetic study of betamethasone dipropionate cream in rabbits. Maximal betamethasone plasma level was observed after (11.0±5.3) h and the Cmax was (167±28) pg/ml,AUC0-t was (4.24±1.68) ng·h·ml-1 after percutaneous administration of 0.5 g betamethasone dipropionate. Conclusion: This method is selective and sensitive,and can be used for the purpose of the pharmacokinetic study of betamethasone dipropionate cream.

Abstract:1临床资料患者女性,22岁,自幼发现心脏杂音,当地诊断“动脉导管未闭(PDA)”,平时易患感冒,稍活动后感心悸、胸部不适,无发绀、肢肿、尿少及不能平卧等症状。入院时查体:血压140/70 mmHg(1 mmHg=0.133 kPa),口唇无发绀,胸部未触及细震颤,心界向左下扩大,心率84次/min,律齐,P2亢进,胸骨左缘第2肋间可闻及4级收缩期吹风样杂音,股动脉可闻及枪击音,四肢温暖,外周毛细血管搏动征阳性。胸片示肺充血、肺动脉段膨隆、左心室扩大。心脏超声示左房内径47 mm,左心室内径(d/s)65/43 mm,轻度三尖瓣反流,估测肺动脉压110 mmHg,PDA直径为10 mm。股动脉血pH 7.44,PCO2 30.5 mmHg,PO2 97.2 mmHg,SO2 97.6%。遂在局麻下行PDA封堵术,将导引钢丝由髂静脉前送时,见其经上腔静脉进入右房和右室,遂行静脉造影,见下腔静脉异位引流,下腔静脉在平第5腰椎水平分出2支,1支经奇静脉汇入上腔静脉;另1支明显扭曲,部分与肝静脉汇合,部分汇入上腔静脉。降主动脉造影示PDA呈管状,直径约10 mm。主动脉压为190/100(120) mmHg,肺动脉压为128/70(48) mmHg。遂穿刺右颈内静脉,建立主动脉-动脉导管-肺动脉-右心室-右心房-颈内静脉轨道。选用9F输送长鞘常规封堵,封堵器(上海形状记忆材料有限公司)大小为16 mm。再次主动脉造影无残余分流,超声显示肺动脉压34/16(22) mmHg,观察20 min,患者无不适感,血压、心率均稳定,遂释放封堵器。撤出输送鞘管时,心率由80次/min减至50次/min,停止操作后逐渐恢复。术毕时经颈内静脉置入导管测肺动脉压为39/17(28) mmHg,主动脉压150/80(90)mmHg。术后第3日复查超声示封堵器周围未见异常血流,左心房内径37 mm,左心室内径(d/s)60/40 mm,剑突下切面见上腔静脉回流至右房,内径较宽,下腔静脉较细小,心尖各切面观可见左房底部有异常管样腔隙,血流呈低速。股动脉血pH7.45,PCO2 34.2 mmHg,PO2 100 mmHg,SO2 98.3%。5 d后患者出院。[第一段]

Abstract:S100A,a Ca2+-depending inotropic factor in the heart,is found decreased during hear failure.Increase of S100A1 protein expression can improve cardiac function by regulating Ca2+ transportation,inhibiting left ventricular remodeling,decreasing apoptosis,and restoring energy supply of failing heart.Therefore,recovery of S100A1 protein expression in the failing heart is an effective strategy for treatment of heart failure.

Abstract:Medial pontine reticular formation (mPRF),an important part of brainstem reticular formation,has drawn great attention from anesthesiologists due to its close relation with not only consciousness, but also analgesia,muscle relaxation and autonomic reflex.Progress in related research may provide evidence for understanding the mechanism of general anesthesia;meanwhile,it may serve as an effective anesthesia target site,which may contribute to improvement in anesthetic effect and reduction of complications.

Abstract:目的:对新型基质(木糖醇与淀粉的低共熔混合物)复方丹参滴丸的制剂工艺进行优化。方法:以滴丸外观质量、硬度、冷凝性、药物含量、溶散时限等为评价指标,对加入方式、药物基质比、冷凝液种类、滴制温度、冷凝液温度、滴距、滴速、化料时间等8个因素进行单因素实验考察;以滴丸成型率为考察指标,进行正交设计试验,筛选出最佳的滴丸成型工艺条件。结果:最佳制剂工艺为主药以喷雾干燥粉末的形式与基质按1︰5.5的比例混合,重质液体石蜡为冷凝剂,药液温度为85~95℃,化料时间为1~2 h,冷凝液温度为-2~4℃,滴距为5~10 cm,滴速为每分钟30~45滴。结论:优化工艺制备的新基质复方丹参滴丸成型性好,质量稳定,重现性好,可为今后的工业化生产提供参考依据。

Abstract:目的:研究国产头孢丙烯胶囊在健康人体内的药物动力学和相对生物利用度。方法:20例男性健康受试者双周期随机交叉单剂量口服500 mg头孢丙烯胶囊与片剂,两次试验间隔为1周,采用HPLC法测定血浆中头孢丙烯浓度。结果:头孢丙烯胶囊与片剂的主要药代动力学参数Cmax分别为(8.78±1.92)和(8.66±1.57) μg/ml;tmax分别为(2.08±0.82)和(1.93±0.61) h;t1/2分别为(1.40±0.267)和(1.40±0.18) h;AUC0-10分别为(28.17±4.31)和(28.28±3.28) h·μg·ml-1,头孢丙烯胶囊的相对生物利用度为(99.61±10.62)%。结论:头孢丙烯胶囊口服后吸收迅速,统计分析结果表明两种制剂具有生物等效性。

Abstract:1临床资料 例1:女性,38岁。因“吞咽固体食物困难1年,近来频发”于2007年6月7日入院。纤维胃镜检查示“食管下段隆起性改变、浅表性胃炎”,经腹部超声和X线CT检查分别提示“贲门部食管壁局限性增厚”和“食管下段占位性病灶”。2007年6月13日以“食管下段肿瘤”在全麻下行“经左胸食管肿瘤切除术”。术中探查发现肿瘤大小约6 cm×5 cm×4 cm,环绕食管近1周,呈生姜样,有包膜;食管旁未见肿大淋巴结;沿肿瘤边缘分离,见肿瘤位于食管黏膜外,与黏膜有部分粘连。分离过程中约0.5 cm食管黏膜破损,将破损的黏膜和浆肌层间断全层缝合,经充气试验确认吻合口紧密缝合;彻底止血后缝合口处再涂抹生物蛋白凝胶,置胸腔引流管。术后病理:食管间质细胞肿瘤。[第一段]

Abstract:1临床资料 患者女,17岁,因“突发左上腹痛、反复发热20余日”于2007年9月11日由急诊收入我科。患者20 d前无明显诱因出现左上腹痛,次日腹痛缓解,出现发热,体温最高40℃,至外院查血常规:WBC 18×109/L,中性79.9%,予阿莫西林抗感染治疗6 d,体温未恢复正常,遂至我院就诊。入院查体:体温37℃,神志淡漠,查体欠合作,智力障碍。面部见对称性淡褐色蜡状丘疹,背部数个鲨革斑。眼底检查未见渗出及血管畸形;心肺无异常;腹稍膨隆,左右均可触及约15 cm×10 cm大小肿块,边界不清,质硬,无触痛,肾区无叩击痛。血常规正常,尿常规见红细胞3~5个/HP。头颅CT示:右侧颞叶片状高密度钙化灶。脑电图提示:轻度异常脑电图,为广泛性改变。腹部B超:肝内见多个实性高回声结节,最大的位于右前叶,大小约2.0 cm×1.5 cm,边界清,内回声不均。B超右肾区见大小15.7 cm×7.9 cm×11.4 cm实性高回声区,边界呈分叶状;左肾区见大小约24.0 cm×6.9 cm×10.0 cm类似回声区,其内见6.6 cm×4.4 cm×3.8 cm无回声区,内透声差伴分隔。静脉肾盂造影:左侧下盏受压上移,变形,但边缘光滑;右侧正常。进一步查腹部CT(图1A)及MRI(图1B、1C)均提示:双肾外形消失,内部呈高低混杂信号,见巨大含脂肪成分肿块影,内见分隔。增强后分隔状强化,含脂部分不强化并可见囊变;脂肪抑制T1W1增强可见肿块明显强化,囊变部分未见强化。肝右叶2枚含脂小结节影,最大约1.5 cm,增强后轻度强化(图1D)。脾脏略增大,脾内小结节影边界欠清,轻度强化,约0.7 cm。既往有癫发作史,伴智力减退,幼时面部出现粉红色蜡状丘疹。诊断为肝右叶及双肾血管平滑肌脂肪瘤,脾脏良性小结节影。本例患者因肿块过大,与正常肾组织分界不清,外科认为已无手术指征,予保守治疗,出院时患者腹痛基本消失,体温、血、尿常规等恢复正常。[第一段]

Abstract:1临床资料 例1:患者男性,38岁。因“双下肢无力、行走困难伴胸背部疼痛进行性加重4年”于2005年6月来我院就诊。患者于16年前因胸椎结核行后路引流术并予以口服抗结核药物治疗。查体:驼背畸形,胸背部活动受限,双侧脐以下感觉减退,下肢肌力3级,括约肌功能正常,下肢腱反射减弱,病理征阴性。X线侧位片示胸椎后凸畸形,Cobb角约65°(图1A);CT矢状位重建显示T7~T10陈旧性结核,合并T8~T10多节段连续型黄韧带骨化(图1B);MRI检查结果显示脊髓同时受到前方后凸畸形及后方黄韧带骨化的双重压迫(图1C)。红细胞沉降率为6 mm/1 h。手术予以后路椎板切除减压及椎弓根螺钉内固定术,术后患者胸背疼痛即刻缓解,随访1年患者下肢肌力恢复至4级,残留左下肢小腿感觉减退。[第一段]

Abstract:1临床资料 患者男性,61岁,因“发现左侧腹股沟肿块增大伴破溃3个月余”于2006年4月11日入院。患者患银屑病30年余,长期应用本院自制皮肤霜、曲安奈德(去炎松A)及复方酮康唑霜等药物外涂,发病初曾行氦氖激光治疗1次(共5 d),近2年来加用环孢素50 mg×2次/d口服治疗,经上述联合治疗后患者全身银屑病症状尚稳定,但皮损一直未能痊愈,时有发作。患者曾使用不明中药治疗。3个月前发现腹股沟肿块,外院予新霉素软膏外用后未有好转,遂至我院作进一步诊治。入院查体:体温36℃,全身皮肤散在斑片状红色斑块,略高于皮面,表面脱屑明显,可见薄膜现象和点状出血。左腹股沟可见皮肤肿块,呈圆形,直径约8 cm,高出皮面约1 cm,顶部呈 “菜花状”,伴破溃,包块周围界限较清楚,但浸润位置较深,活动度小,色粉红,质硬,压痛明显,周围皮肤略红肿,无卫星包块(图1)。左侧腹股沟区可触及数个肿大淋巴结,约2 cm×1 cm大小,质韧,与周围组织粘连,活动度较差,有触痛,余浅表淋巴结未及肿大;其他系统检查未见明显异常。家族中无银屑病及肿瘤病史。实验室检查结果:血、尿、粪三大常规,肝肾功及电解质均未见明显异常。心电图:窦性心律,偶发室性早搏,Q-T延长;B超:左侧腹股沟淋巴结肿大(较大约23 mm×14 mm,部分淋巴门结构不清),左侧腹股沟低回声包块(29 mm×20 mm,内部回声不均匀);胸片未见明显异常。诊断:左腹股沟皮肤鳞状细胞癌(2级),红皮病型银屑病。入院后继续给予本院自制皮肤霜、咪康唑(达克宁)乳膏等药物全身外涂,环孢素50 mg×2次/d口服,治疗后斑块颜色变浅,瘙痒症状减轻。完善检查后于2006年4月28日全麻下行左腹股沟皮肤肿块切除,左上腹取全厚皮覆盖遗留皮肤缺损、行全厚皮片移植术。术中见癌肿呈浸润生长,皮下筋膜、血管及淋巴管解剖不清。术后给予加强换药、补充白蛋白等对症支持治疗1个月,植皮成活,创面基本愈合。术后皮肤肿块病理结果:肉眼见灰白色肿物,切面见分叶结节状,表面累及皮肤;病理诊断“左腹股沟”鳞状细胞癌(中分化);免疫组化:c-erbB-2(-)、Ki67(20%~50%阳性)、p53(+)、CEA(-)、nm23(+)、高分子细胞角蛋白CKH(+)、低分子细胞角蛋白CKL(弱阳性)、 EMA(+)。详见图2。[第一段]
