Abstract:Objective To investigate the specific cytotoxicity of MUC1-specific chimeric antigen receptor (CAR)-engineered Jurkat T cells against hepatocellular carcinoma (HCC) cells. Methods The expression cassettes of both the first and the third generation MUC1-specific CAR gene (i.e. MUC1-CAR and G3MUC1-CAR) were constructed and cloned into lentivirus transfer plasmids, respectively. Then the obtained recombinant lentiviruses carrying the MUC1-specific CAR gene and hrGFP reporter genes were used to infect Jurkat T cells in vitro to establish MUC1-sepcific CAR-engineered Jurkat cells (i.e. CAR-T cells). Subsequently, the assays of CCK-8, ELISA, and LDH were used to detect the cell proliferation, IL-2 secretion and killing effect of the CAR-T cells, respectively. Results We successfully constructed the expression cassettes of MUC1-sepcific CAR gene and the corresponding recombinant lentivirus, established the MUC1-specific CAR-engineered Jurkat T cells. Both types of MUC1-specific CARs-engineered Jurkat T cells could recognize MUC1 molecule and specifically kill MUC1 over-expressed HCC cells while left normal hepatic cells almost undamaged. In addition, the cell proliferation, the secretion of IL-2 and killing effect of the G3MUC1-CAR-engineered Jurkat T cells was significantly superior to the MUC1-CAR-engineered counterpart(P<0.05 or 0.01). Conclusion The MUC1-sepcific CAR-engineered Jurkat T cells can specifically kill MUC1 over-expressed HCC cells.

Abstract:Objective To construct the lentivirus vector targeting human protein kinase R-like endoplasmic reticulum kinase (PERK) gene, and to study the effect of lentivirus modification of human PERK gene on endoplasmic reticulum(ER) stress-mediated apoptosis in C2C12 cells. Methods Primer pairs of PERK gene were designed and synthesized. Human PERK gene was amplified by PCR from the expression plasmid pCDNA3.l (-)-PERK and was cloned into pWPT-GFP lentivirus vector. Then the recombinant PERK lentivirus were produced in 293T cells following co-transfection between pWPT-GFP-PERK and the lentivirus packaging plasmids pMD2G, pSPAX2. The supernatants were collected 48 h after transfection and myoblast C2C12 were infected in vitro. Infection efficiency of green fluorescent protein (GFP) was observed. The effect of recombinant lentivirus PERK on proliferation and apoptosis in C2C12 cells was detected by flow cytometry (FCM) in ER stress. Western blotting analysis was used to examine the expression of apoptosis-related proteins Cleaved Caspase-3 and Chop. Results The recombinant lentivirus PERK was successfully constructed, with a titer of 4.2×108 efu/mL. FCM analysis showed that under ER stress condition, the ratio of G1 phase C2C12 cells in the TM+LV-PERK group (54.37 %) was significantly lower than those in the TM group (77.91%) and LV-GFP (66.41%, P<0.05); the ratio of S phase C2C12 cells in the TM LV-PERK group (31.36%) was significantly higher than those in the TM group (12.14%) and LV-GFP (16.96%, P<0.05). The apoptosis rate in the TM LV-PERK group(25.91%) was significantly higher than those in the TM group (11.79%) and Ad-GFP group (11.24%, P<0.05). Western blotting analysis showed that the expression of Cleaved Caspase-3 and Chop protein was consistent with the results of FCM. Conclusion We have successfully constructed and packaged recombinant lentivirus PERK. Under ER stress condition, LV-PERK can promote the proliferation and apoptosis in C2C12 cells.

Abstract:Objective To investigate the relationship between oncogenic H-RasV12 overexpression/activation and the autophagic activity by observing the effect of Ras overexpression on autophagic activity in human fibroblast cells. Methods Human BJ fibroblast cells were transfected with H-RasV12 or control vector, and then the cellular responses to H-RasV12 overexpression were analyzed by observing the morphology, cell growth curve, senescence-associated β-Gal staining, Western blotting analysis, flow cytometry, and suppression of autophagy-related protein 5 (ATG5) by siRNA. Results Compared with control group, BJ cells overexpressing H-RasV12 developed prominent premature senescence and inhibited autophagic activity, as manifested by significant accumulation of p62 and light chain 3Ⅱ (LC3Ⅱ). The autophagy inhibition by H-RasV12 remained stable during the study period; the apoptosis rate was increased in H-RasV12 overexpressing BJ cells compared with that in the control cells. Suppression of ATG5 by siRNA led to more severe senescence in Ras-overexpressing BJ cells. Conclusion Our results suggest that the autophagy activity is inhibited in human fibroblast cells stably overexpressing oncogenic H-RasV12, and the inhibition is in the later stage of autophagy, which may be related to H-RasV12-related tumorigenesis.

Abstract:Objective To observe the effects of selegiline on tyrosine hydroxylase (TH) and neuronal nitric oxide synthase (nNOS) in the gastric antrum of rats with Parkinson disease(PD), so as to investigate the therapeutic effect and mechanism of selegiline for treatment of gastric disfunction in PD. Methods A total of 72 SD rats were randomly divided into normal control group, PD model group, and selegiline treatment group. PD rat models were established by subcutaneous rotenone injection. After the success of model preparation, PD group was given normal saline, and the therapy group was given selegiline (0.5 mg/kg) by intragastric administration. At 4 d and 8 d after therapy, residual rate of solid food was detected in the stomachs of animals, and the expressions of TH and nNOS were detected by immunohistochemistry method and Western blotting analysis. Results Compared with the control group, the model group had significantly higher residual rate of solid food, significantly lower TH expression, and significantly increased nNOS expression in the gastric antrum (all P<0.01). Compared with the model group, the treatment group had significantly decreased residual rate of solid food, significantly increased TH expression and significantly lower nNOS expression in the gastric antrum at all time points(P<0.05,or P<0.01). At 8 days after selegiline treatment, the residual rate of solid food was significantly lower, TH expression was significantly increased, and nNOS expression was significantly decreased (all P<0.05) compared with those at 4 days after treatment. Conclusion Selegiline can improve the gastric dysfunction in PD rats, which might be related to relieve of gastric dopaminergic neuronal injury and inhibition of nNOS expression.

Abstract:Objective To investigate the expression of insulin-like growth factor 1 (IGF-1) in epidermal growth factor receptor tyrosine kinase inhibitors (EGFR-TKIs) resistant non-small cell lung cancer (NSCLC) cell lines and patients, and to discuss the relevant clinical significance. Methods Levels of IGF-1 were measured by ELISA kit in PC9, A549, H1975 cells and sera of EGFR-TKIs treated NSCLC patients; insulin-like growth factor-binding protein (IGFBP) expression was examined by Western blotting analysis and immunocytochemical method. Cell growth of NSCLC cells treated with gefitinib and BMS536924 (antagonist of IGF-1 receptor) was determined by MTT. The clinicopathological characteristics and survival period of 84 patients receiving EGFR-TKIs treatment were collected and their relationship with IGF-1 expression was analyzed. Results It was found that IGF-1 level in sensitive cell line PC9 was significantly higher than those in EGFR-TKIs resistant cell lines A549 and H1975 (P<0.05), while IGFBP expression was lower than those in A549 and H1975 cells. Addition of BMS536924 of different concentrations significantly promoted the inhibitory effect of gefitinib against growth of A549 and H1975 cells (P<0.05).The patients with disease progression showed higher IGF-1 expression than those without disease progression (P=0.052),while the patients with intrinsic resistance to EGFR-TKIs had significantly higher IGF-1 level than those with acquired resistance (P=0.02). The patients with IGF-1 negative expression had significantly longer survival than those with positive expression(P=0.002). Conclusion IGF-1 expression is detected in EGFR-TKIs resistant NSCLC cell lines and patients, and antagonist of IGF-1 receptor can restore the sensitivity to EGFR-TKIs. The expression of IGF-1 is also a prognostic factor of NSCLC patients.

Abstract:Objective To sequence the ATP7B gene in patients with Wilson's disease (WD) and to analyze the relationship between the mutations and WD. Methods The genomic DNA was obtained from the oral mucosal cells of 67 clinically diagnosed WD patients; PCR was used to amplify all the exons 5' end→ 3' end of ATP7B gene. And the PCR products were subjected to DNA direct sequencing for mutations. Results We found that the ATP7B gene mutation rate was 77.61%(52/67) in WD patients. Of these patients,16 had homozygote mutations (including 12 patients with Arg778Leu and 4 with Arg919Gly), 5 had complex mutations, and 31 had simple hetrozygote mutations. Five types of the ATP7B gene complex mutations were rarely reported in China. Conclusion We have identified 5 complex mutations of ATP7B gene, which might be related to the development and progression of WD and deserves further study.

Abstract:Objective To explore the changes of HSP70 expression after radiofrequency ablation of benign thyroid nodules and to discuss the potential relevance. Methods Ultrasound-guided radiofrequency ablation (RFA) was conducted for treating 50 solid thyroid nodules in 50 patients. Three different regions of interest (ROI), namely the central region, marginal region and transitional region, were set on each nodule for quantitative investigation of HSP70 expression by using immunohistochemistry method. Each ROI experienced histological sampling by using core-needle biopsy before and after RFA. HSP70 expression was compared for the same ROI before and after ablation and between different ROIs after ablation. Results HSP70 expression had no significant changes in the central and marginal regions before and after ablation in the 50 solid thyroid nodules, and the expression was significantly enhanced in the transitional region after ablation (P<0.05). After ablation the expression of HSP70 in the transitional region was significantly higher than those in the central and marginal regions (P<0.05), with no significant difference found between the latter two regions. Conclusion Increased expression of HSP70 in the transitional region might protect normal glandular tissue against heat damage during thermal ablation treatment of benign thyroid nodules.

Abstract:Objective To know about the ongoing pregnancy rate, cesarean section rate of HIV-infected pregnant women and formula feeding rate of their infants in China during 2003-2011. Methods Databases including PubMed, Web of Science, the Cochrane Library, China National Knowledge Infrastructure (CNKI), Chinese Science and Technology Periodical Database (VIP), Wanfang Database (WF) and Chinese Biological Medical Literature Database (CBM) were searched from the date of their establishment to May 2013; the literatures published in both Chinese and English reporting ongoing pregnancy, cesarean section of HIV-infected pregnant women, or formula feeding of their infants were collected. Then according to the inclusion criteria, two researchers independently screened the literature, extracted the data and assessed the quality of the included articles. Meta-analyses were conducted using the Comprehensive Meta-Analysis software (V2.0, Biostat, Englewood, New Jersey). Meta regression analysis was conducted to identify the source of heterogeneity. Results Of the total 2 356 records, 61 eligible articles were finally included. The results of meta-analysis showed that nationally, the rate of pregnancy continuation rates among HIV-infected pregnant women during 2003-2011 were 67.50%(95%CI[51.73%, 80.10%]),60.49%(95%CI[18.59%, 91.13%]),51.80%(95%CI[28.13%, 74.68%]),62.59%(95%CI[54.60%, 69.96%]),64.93%(95%CI[50.18%, 77.29%]),70.65%(95%CI[62.20%, 77.88%]),65.66%(95%CI[59.70%, 71.16%]),67.85%(95%CI[52.66%, 80.02%]),and 75.00%(95%CI[59.46%, 85.99%]), respectively; the caesarean section rates among these women during 2004-2010 were 26.33%(95%CI[9.41%, 55.14%]),43.40%(95%CI[34.30%, 52.96%]),42.57%(95%CI[35.73%, 49.70%]),69.43%(95%CI[13.48%, 97.07%]),46.68%(95%CI[27.27%, 67.16%]),61.14%(95%CI[49.37%, 71.75%]) and 56.60%(95%CI[36.36%, 74.85%]), respectively; and the formula feeding rates of exposed infants were kept at relatively high levels (90.00% above) for all the years except for 2005, which was 82.65%. Conclusion The overall rate of pregnancy continuation of HIV-infected pregnant women represents an increase in recent years in China; the cesarean section rate is high, fluctuating and increasing; the formula feeding rate of their infants keeps stable at a high level.

Abstract:Objective To assess the therapeutic strategies and efficacy of moyamoya disease of different clinical types and radiographic stages. Methods The clinical types, radiographic stages and treatment strategies of 39 patients with moyamoya disease, who were treated in our medical centers from April 2010 to December 2013, were retrospectively analyzed. The treatment efficacies were analyzed by CT angiography, CT perfusion and DSA. The average follow-up time was (21.3±3.1) months (range: 7-44 months). Results Thirty-four patients had type Ⅰ-Ⅴ moyamoya disease, including 6 treated by encepho-duro-myo-synangiosis (EDMS) and 28 by STA-MCA anastomosis combined with EDMS, with 4 receiving a second surgery on the other hemisphere 6 months after first surgery. Five patients with type Ⅵ moyamoya disease underwent emergency surgery: cerebral hematoma evacuation+decompressive craniectomy+EDMS. Stage Ⅰ and Ⅱ patients were treated by conservative therapies. Stage Ⅲ-Ⅵ patients were treated by EC-IC cerebrovascular reconstruction. The skin incision, bone window, and dura incision were designed individually in order to protect anastomoses formed pre-op in stage Ⅴ & Ⅵ patients. All patients had improved clinical condition during follow-up. CTP imaging showed significantly increased relative cerebral blood flow and relative cerebral blood volume, and decreased time-to-peak after the operation(P<0.05), and these changes were intensified with the prolong of follow-up. Conclusion Selection of appropriate treatment strategy according to different clinical types and radiographic stages can achieve better therapeutic efficacy for moyamoya disease patients.

Abstract:Objective To observe the effect of obstructive jaundice on the neuromuscular blocking effect and drug metabolism of rocuronium, so as to guide proper use of rocuronium during anesthesia for patients undergoing hepatobiliary surgeries. Methods Thirty patients (with normal renal function and without muscle or nerves diseases, ASAⅠ-Ⅱ) receiving elective surgery were divided into 2 groups: obstructive jaundice group (group Ⅰ, n=15) and control group (group Ⅱ, n=15). All patients received continuous peridural anesthesia and general anesthesia. Neuromuscular blockade was evaluated with train-of-four (TOF) stimuli of the ulnsar nerve using TOF Watch SX nerve stimulators. The onset time, clinical duration of rocuronium, the time of TOFR from 0 to 70% and recovery index (T1 from 25%-75%) were observed. Liquid chromatography-tandem mass spectrometry (LC/MS/MS) was used to determine the concentration of rocuronium in the plasma at 30, 60, 90, 120, and 180 min after administration of 0. 9 mg/kg rocuronium. Results The age, body mass index, and prothrombin time were matched between the two groups. The total bilirubin, direct bilirubin and indirect bilirubin in groupⅠwere significantly higher than that in group Ⅱ(P<0.01) . The onset time was similar between the two groups; the clinical duration of rocuronium, the time of TOFR from 0 to 70%, and the recovery index (to 10%, 25%, 50%, and 75% of T1) in groupⅠwere significantly longer than those in groupⅡ(P<0.01). The plasma concentrations of rocuronium in groupⅠwere significantly higher than those in group Ⅱat 60, 90, 120 min after administration of rocuronium (P<0. 01 or P<0.05). Conclusion Obstructive jaundice may prolong the duration of rocuronium action and recovery time, and slows down the metabolism of rocuronium. So patients with obstructive jaundice should have a longer interval for drug addition. Peri- and post-operative monitoring of neuromuscular function is suggested for proper drug use and anesthesia extubation.

Abstract:Objective To establish a HEK-293T cell model with sodium channel function similar to that in PRKAG2 syndrome. Methods Vectors of PRKAG2 gene and SCN5A gene were constructed and were used to co-transfect HEK-293T cells. Immunofluorescence, real-time PCR and Western blotting analysis were used to examine the transfection results and the expression of interested mRNA and protein. Results Imunofluorescence findings showed red fluorescence and green fluorescence in HEK-293T cells 48 hours after co-transfection. Real-time PCR and Western blotting analysis showed SCN5A expression in pure SCN5A group, SCN5A + wild-type group, SCN5A + R302Q mutant group, and SCN5A + G100S mutant group, but not in the blank control group, showing significant significance(P<0.05). Expression of PRKAG2 gene and protein in SCN5A+PRKAG2 group was significantly higher than those in the blank control group, pure SCN5A group, SCN5A+R302Q, and SCN5A+G100S groups (P<0.05) . Conclusion We have successfully established a HEK-293T cell model with sodium channel function similar to that in PRKAG2 syndrome, paving a way for future study.

Abstract:Objective To conduct a integrated analysis of gene expression signature and gene expression profile, so as to provide reference for the prognosis and drug-resistance of serrated colorectal adenocarcinoma (SCA). Methods We downloaded four gene expression datasets (GSE14333, GSE17538, GSE33113, and GSE37892) of colorectal carcinoma with the follow-up survival data from GEO database, and then integrated them into a entire expression profile (n=600) with batch adjustment and gene expression value extraction. An SCA gene signature and the corresponding expression profile were integrated with the previous expression dataset. Using the gene signature score model we assigned score to each patient in the gene expression dataset and classified the patients into serrated, transitional, or conventional subtypes. Kaplan-Meier analysis and Cox model were used to compare the risks of cancer recurrence between three subtypes of colorectal carcinoma. Gene signature model were also used to generate the score for each patient in the datasets associated with alleviative therapy of colorectal cancer(GSE28702, GSE5851). The SCA drug-resistance was analyzed by observing the therapeutic effective group and non-effective group. Results According to cut-off value of CRC subtypes, 600 patients in the combined dataset were classified as 50 with serrated subtype, 126 with transitional subtype, and 424 with conventional subtype. Survival analysis showed the serrated and transitional subtypes had very similar scores to predict patient survival, and they were also independent risk factors for postoperative recurrence.When comparing serrated and conventional subtypes, multivariate Cox model analysis indicated the patients with serrated subtype was an unfavorable independent risk factor for prognosis(AHR=1.792, 95%CI 1.011-3.177). Responders to cetuximab treatment had significantly higher signature scores than non-responders (P=0.017), while responders to FOLFOX treatment had similar signature scores with the non-responders. Conclusion Serrated subtype is an independent risk factor of postoperative recurrence in SCA patients, and is related to the treatment with cetuximab.

Abstract:Hypertensive disorder complicating pregnancy is a commonly seen pregnancy complication that may pose serious problem to maternity and infant health, and is one of the major causes for morbidity and mortality in pregnant women and neonates, with the specific etiology and pathogenesis remain unclear. Recent studies have found that sympathetic nervous activity hyperthyroidism plays a role in the pathogenesis of hypertensive disorders in pregnancy. This review focused on the relationship between the sympathetic nerve activity and hypertensive disorder complicating pregnancy.

Abstract:Hepatic fibrosis is a common disease caused by wound-healing response to a variety of chronic liver injuries; it is characterized by the imbalance of extracellular matrix synthesis and degradation. Angiotensin Ⅱ is the major effector of the renin-angiotensin system. Increasing evidence has demonstrated that the interaction of angiotensin Ⅱ with angiotensin receptor 1 plays an important role in the long-term liver injury-induced liver fibrosis by inducing the activation,proliferation and constriction of hepatic stellate cells, generation of the reactive oxygen species from activated hepatic stellate cells, and the synthesis and accumulation of collagen. In this article we focused on the role of angiotensin Ⅱ and its receptor 1 in the pathogenesis, development and therapy of liver fibrosis in recent years.

Abstract:Colon cancer stem cells(CCSCs)play key roles in tumor initiation, proliferation,metastasis,and chemoresistance. Currently a large number of reports focused on markers of CCSCs, with relative less reporting on CCSCs biological characteristics: proliferation, metastasis, and chemoresistance. This paper reviewed the current research on the biological behavior of CCSCs, with special attention paid on the regulatory effects of microRNAs on CCSCs, hoping to provide reference for improving the diagnosis and treatment of colon cancer.

Abstract:Objective To establish an ultra-fast liquid chromatography (UFLC) method for detecting the substances in different preparations of nimesulide. Methods Ultra-fast liquid chromatography instruments Shimadzu and Syncronis C18 column (100 mm×2.1 mm×1.7 μm) were used. The mobile phase was a mixture solvent of methanol-acetonitrile-0.1% solution of orthophosphoric acid (adjusted to pH 6.5 with ammonia)(45:15:40). The flowing rate was 0.3 mL/min,and the detection wavelength was 230 nm. The system suitability solutions of European Pharmacopoeia and Chinese Pharmacopoeia, the damaging samples and the accessories of different preparations of different manufacturers were used to investigate the method applicability. Results The method had high resolution, strong specificity and can efficiently and quickly detect the related substances in nimesulide preparations.Various degradation products of damaging tests and the accessories of different preparations from different manufacturers had no effect on the determination results. Conclusion The method is efficient, fast, widely applicable and can be used for detecting the substances in different preparations of nimesulide by different manufacturers.

Abstract:Objective To investigate the effect of CrkL silence on hypoxia/reoxygenation (H/R)-induced apoptosis and survival inhibition in cardiomyocytes and the underlying mechanisms. Methods H9C2 cardiomyocytes were infected with blank, negative lentivirus and CrkL RNAi lentivirus (CrkL mRNA silence), and then treated with H/R separately. The cardiomyocytes were divided into blank group, negative lentivirus group, CrkL silence group, blank + H/R group, negative lentivirus H/R group and CrkL silence H/R group. The expression of CrkL mRNA was detected by RT-PCR, and the expression of CrkL and p-ERK1/2 protein was detected by Western blotting analysis. The apoptosis rate of cardiomyocytes was analyzed by flow cytometry, and the cell proliferation rate was analyzed by MTT. Results Compared with blank and negative lentivirus groups, CrkL silence group had significantly decreased CrkL mRNA and protein, p-ERK1 protein, p-ERK1/2 protein and proliferation rate(P<0.05 or P<0.01), and significantly increased apoptosis rate(P<0.01). The same was true for CrkL silence H/R group when compared with the blank H/R and negative lentivirus H/R groups. Conclusion CrkL silence can aggravate H/R-induced apoptosis and survival inhibition in cardiomyocytes, which might be mediated by the decrease of p-ERK1/2 protein expression.

Abstract:Objective:Through the 1:1 matched case-control study to investigate the relationship of thyroid-associated ophthalmopathy with type A personality and analyzed the risk factors of thyroid-associated ophthalmopathy.Methods:From 2013 October to 2014 February in Department of ophthalmopathy of Shanghai Changzheng Hospital outpatient and inpatient diagnosis of thyroid-associated ophthalmopathy and the age is 18 years old to 65 years old primary patients and healthy patients, a total of 201.The unity of "Psychology of personality" in 2012 August, the first edition of the first printed in line with China"s national conditions of the Type A Personality Questionnaire survey, Remove the Polygraph questions the L greater than or equal to 7 of the 81 questionnaires, collected 60 cases of thyroid-associated ophthalmopathy patients, and 60 healthy controls were enrolled a total of 120 cases.Two groups of patients by age, culture level (high school and above), the marriage 1:1 paired design, inclusion sex (female), family history of thyroid-associated ophthalmopathy, type A personality types as risk factors, using the mult-factor conditional Logistic regression analysis of case-control study.Result:the results of Multi factor Logistic regression analysis of thyroid associated ophthalmopathy and type A personality of showed that women, family history of Graves disease is a risk factor for thyroid associated ophthalmopathy, according with the literature,multivariate analysis showed that Type A personality OR value was 4.571 (90%CI:1.912-10.926), the P value is 0.0041, suggesting that the existence of the obvious relationship between the type A behaviour personality and thyroid associated ophthalmopathy.Combined with the results of single factor analysis, the TH (time hurry: time is pressing type) factors have obvious influence on thyroid associated ophthalmopathy, the value of OR was 1.126 (95%CI:1.029-1.233).Type A behaviour pattern was shown to be an independent risk factor for the development of TAO. Conclusion:the type A behaviour personality is conducive to thyroid associated ophthalmology prevention and rehabilitation.A personality, women and family history of thyroid associated ophthalmopathy were risk factors for thyroid associated ophthalmopathy, the type A personality especially TH (time urgency) is an important risk factor in the incidence of thyroid associated ophthalmopathy.People with type A personalities should be adjusted over time urgency, keeping a good mood is conducive to thyroid associated ophthalmology prevention and rehabilitation.

Abstract:183Background More clinicalSdoctors tend to choose minimally invasive treatments for patella fracture. Because patients have lower complications,better function and perfect incision,which makes them early continue working and influences a little on their careers and life. Objective To explore the efficacy of minimally invasive treatment of patella transverse of young people, we choose percutaneous cannulated screw and titanium cable that we design to operate surgery .we observe the surgical incision,hemorrhage and function Methods From February 2009 to June 2012, we applied the minimally invasive technique of percutaneous cannulated screw and titanium cable to treat 30 cases of patella transverse,16 males and 14 females. Aged 24-45years,mean 35.4years.injury mechanism:10 cases in the traffic accident,6 cases in the fall s injury, 14 cases in the kneeling injury. fracture type:6 cases proximal pole fracture ,11 cases middle pole fracture and 13 cases distal pole fracture. Result All patients healed incision. 30 cases were followed up for mean 18 months. Fracture healed in mean 6.8 weeks. According to Lysholom function score, excellent in 27 cases, good in 2 cases, good rate of 100%. Conclusion Minimally invasive technique of percutaneous cannulated screw and titanium cable to treat the patella transverse fracture is the better choice. I t fixes reliable ,follow the biomechanical and have low surgical damage. Patients have less complication and get a good clinical efficacy. It is a better choice to treat patella transverse fracture of young fracture.

Abstract:Objective To observe the effect of lidocaine aerosol on preventing cardiovascular response and sore throat, hoarseness in the patients with double lumen tube to orotracheal intubation. Methods 60 selective patients (ASAⅠ-Ⅱ) scheduled for lung lobectomy were randomly divided into 3 groups, with 20 cases in each group. Lidocaine aerosol (group A) and Saline as a control (group B and group C) were given to spray. After routine induction, the glottis was exposed and 3 press lidocaine aerosol were given in group A, while group B and C were given saline. Patients in group C were received additional 0.5mg/kg of propofol and 1μg /kg of fentanyl intravenously before the exposure of glottis. HR, SBP, DBP, HR*SBP were monitored and recorded at the time points of pre-induction (T0), immediately pre-intubation (T1), 1 min (T2), 3 min (T3), 5min (T4) , 10min(T5) after intubation. Cases of sore throat and hoarseness were also recorded at the point of 1h, 6h, 24h postoperation. Results Compared with T0, values of HR, SBP and RPP at T1 were significantly decreased after induction(P<0.05). Values of HR, SBP ,RPP in group B and C were higher than that in group A at T2 and T3(P<0.05). SBP was significantly decreased at T4 and T5 in group C(P<0.05).Cases of postoperative sore throat and hoarseness in group A were significantly lower than that in group B and C(P<0.05). Conclusion Lidocaine aerosol can effectively suppress cardiovascular responses caused by double lumen tube intubation, decrease the dose of induction drugs, reduce the incidence of postoperative sore throat and hoarseness.
