  • 1  Solitaire stent placement for treatment of complex intracranial arterial stenosis: a preliminary result

    Abstract:
    Objective To evaluate the feasibility, safety and efficacy of Solitaire stent placement for treatment of complex intracranial atherosclerotic stenosis. Methods We retrospectively reviewed the clinical data of 18 patients who underwent Solitaire stent placement for symptomatic intracranial arterial stenoses in our department between November 2011 and June 2012, with focus on the stenosis morphology, condition of the diseased blood vessels, surgical success rate, and perioperative complications. We also summarized the early outcomes and imaging findings during follow-up. Results The patients included 8 males and 10 females, with a mean age of 66.55±9.84 years old. Mori typing results were 5 A, 8 B, and 5 C types; LMA typing results were good in 2, moderate tortuosity in 12, severe tortuosity in 4 cases. All the patients received successful Solitaire stent implantation. The average stenosis rate was reduced from (74±12.5)% to (10±5.5)% after the procedure. There was only one patient (5.6%) had post-operative complication (cerebral infarction of the target vessel territory),who had temporal symptom and recovered well at follow-up.Clinical follow-up was available for 18 patients, with a mean of 6.2± 1.4 months; and no patients experienced any ischemic episodes. Fifteen patients had DSA or CTA follow-up for a mean of 7.8±2.6 months. There were 3 in-stent restenosis (20%) cases and one of them was symptomatic with transient ischemic attack. Conclusion Solitaire stent placement is feasible, safe and effective for complex symptomatic intracranial artery stenosis, with definite short-term effectiveness, and the long-term effect warrants further investigation.

    In recent years, researchers have discovered that NADH and NADPH have important functions in physiological and pathological conditions. In this paper we discussed the synthesis and degradation of NADH and NADPH, focused on the function of NADH, NADPH and NADPH oxidase, and described their functions in various pathological conditions such as inflammation, cardiovascular disease, cancer, neurodegenerative diseases. Nowadays, the functions and metabolism of NADH (NADPH) have evoked international research interest; and the underlying mechanism will be better understood.

    目的:观察创伤性脑水肿合并海水淹溺后大鼠血清、脑组织、肺组织IL-1β和TNF-α含量的变化,探讨其变化规律及临床意义。方法:脑侧方液压打击伤加气管内灌注海水建立大鼠创伤性脑水肿合并海水淹溺模型。伤后1、6、12、24及48 h检测大鼠脑、肺组织含水量及脑、肺组织匀浆和血清中IL-1β、TNF-α的含量;观察伤后24 h脑、肺组织病理学变化。以颅脑创伤合并淡水淹溺、单纯颅脑创伤、单纯海水淹溺、假手术大鼠作为对照。结果:脑损伤合并海水淹溺后,大鼠脑组织含水量较单纯脑损伤、单纯海水淹溺以及脑损伤合并淡水淹溺均显著增高(P<0.05);脑组织、肺组织IL-1β和TNF-α含量有显著变化。脑、肺组织病理学损伤明显。结论:海水的损伤性作用是重要致伤因素之一,炎症反应在创伤性脑水肿合并海水淹溺性肺水肿的病理生理过程中起了重要作用。

    Abstract:
    On Mar. 17, 2023, Annual Sleep Report of China 2023 (abbreviated as Report) was released in Beijing. The Report collected the data of 6 343 people in 2022, and conducted a systematic research on sleep problems from the perspectives of sociology and social psychology, reporting the sleep status of different groups, the relationship between sleep and quality of life, mental health, well being, and social mentality. The Report found that the boundaries between work and sleep are becoming more blurred, which exacerbates the differences between people in sleep time, sleep duration, and sleep quality, leading to inequalities in sleep and health. The Report has a novel research perspective and a large sample size, while it lacks reliable measurement tools and needs further intervention and longitudinal research.

    ObjectiveTo observe the expression of Toll-interacting protein (Tollip) in appendix and analyze its significance during acute inflammation.MethodsThirty-three patients with acute appendicitis were included in the present study and 6 subjects with non-inflammatory appendixes were taken as controls (without inflammatory changes).The pulse rate,body temperature (BT),white blood cell (WBC) count and neutrophil (NEUT) count were observed one hour before appendectomy.Based on H-E staining and pathological examination,the appendix samples were divided into four groups:non-inflammatory appendix (A),simple appendicitis (B),suppurative appendicitis (C) and gangrenous appendicitis (D).The expression of Tollip protein (the localization,qualitative and semiquantitative analysis) was analyzed using immunohistochemistry and digital image analysis.The correlation of Tollip with pusle rate,BT,WBC and NEUT was also analyzed.ResultsSignificant differences in BT,WBC and NEUT were found between different pathological groups (P<0.05).Tollip protein was mainly expressed in the epithelium mucosa and glandular epithelium of appendix,but was not detected in the neutrophils.During the development of appendiceal inflammation (non-inflammatory appendix to simple appendicitis,to suppurative appendicitis,then to gangrenous appendicitis),the expression of Tollip protein gradually increased and was significantly correlated with WBC (P<0.05).ConclusionDuring the development and progression of acute appendicitis,Tollip expression is up-regulated in appendix tissues and is closely correlated with systemic responses of inflammation,such as the increase of WBC.

    目的:探讨主动脉夹层致猝死的临床病理特点、发病机制及危险因素,提高其早期诊断及防治水平,降低病死率。方法:对2002-2007年7例主动脉夹层猝死病例进行临床尸检病理分析,总结其临床病理特点,分析发病机制及危险因素。结果:7例主动脉夹层猝死患者50岁以上4例,40岁以下3例,年龄30~71岁,平均(50.5±15)岁;多有主动脉粥样硬化和(或)高血压病史;临床表现最初大多为非心血管疾病症状。结论:主动脉夹层致病因素复杂,起病隐匿,常导致猝死;伴有长期高血压病、主动脉粥样硬化等高危人群出现突发性胸痛或休克表现时应警惕该病的发生并进行早期干预处理,降低患者病死率。

    The discovery of circulating fetal DNA paves a new way for non-invasive prenatal diagnosis. Examining the specific fetal DNA sequence of the circulating fetal DNA has been used for prenatal diagnosis of sex-linked disorders, fetal rhesus D blood typing and single gene inheritance disease. The variation of the circulating DNA levels can also be used for diagnosis of preeclampsia, premature delivery, and fetal chromosome disorders. This paper aims to review the biochemistry characters and clinical application of the circulating fetal DNA in maternal peripheral plasma.

    Demyelination disables saltatory conduction and leads to loss of neural functions. Remyelination is mediated by oligodendrocyte precursor cells (OPCs), which are widely distributed throughout the adult CNS. A better understanding of oligodendrocyte biology, myelination, and maintenance of myelin sheaths, and the failure of remyelination in the CNS, which has been associated with the limited availability, migratory capacity and myelinating ability of endogenous OPCs, is necessary to improve remyelination strategies in a chronic inflammatory demyelinating disease such as multiple sclerosis (MS).

    [Abstract] Objective To study the discharge change of CA1 place cell after hippocampal formation(HF)accepting the visual-vestibular-proprioceptive mismatch configuration as a match by learning, which could provide new scientific experimental data for revealing the fact that HF may encode any combination of sensory inputs. Methods The visual-vestibular-proprioceptive mismatch configuration was set up. According to theta rhythm recording and theta power calculation in rat hippocampal dentate gyrus, it was acquainted that the rat had been adaptive to the sensory conflict. The assembly discharge of CA1 area place cell was recorded in awake rat by using extracellular tungsten microelectrode record and stereotaxic techniques after the rat adapted to sensory mismatch. Results Out of 56 place cells, 14 showed consistent spatial firing in both the match and mismatch condition (both translocation-related experience-independent neurons). 33 showed different spatial firings across the sessions (both translocation-related experience-dependent neurons). The mean sizes of the place fields were larger in both translocation-related neurons than that of experience-dependent neurons. The firing rate distributions in the place fields were negatively skewed and asymmetric. Conclusion The HF encodes a naturally impossible new configuration of sensory inputs after adaptation, the HF is capable of updating it stored memory to accept a new configuration as a match. The HF may encode any combination of sensory inputs.

    Abstract:
    Training a large number of qualified general practitioners and promoting the hierarchical medical system are key measures to deepen the reform of the medical and health system and implement the “Healthy China Strategy”. After more than 30 years of development, China’s general practice has made significant progress in the cultivation of a talent team, the establishment of a training system, the exploration of service models, and the cultivation of scientific research. However, China’s general practice started late and has a poor foundation. At present, it still faces challenges: quantity and quality of talents need to be improved, the teaching staff and training mode need to be optimized, the hierarchical medical system is not fully implemented, the application channels of research projects are not smooth, and the career attraction is low. Multiple measures should be taken to improve the quantity and quality of general practitioners, to guide the public opinion on seeking medical treatment, to set up more specific funds of general practice to expand the scientific research of general practice, and to reform the incentive and promotion mechanism to enhance the career attraction, so as to promote the sustainable development of general practice.

    Objective:To investigate the reliability, validity and sensitivity of Chinese scale of clinical neurological deficit of stroke patients (China Stroke Scale, CSS), so as to assess its clinical application value.Methods: A total of 126 consecutive inpatients with acute stroke onset were included in our study and they were scored by CSS and the United States National Institutes of Health Stroke Scale (NIHSS) score separately; the reliability, validity, and sensitivity of CSS were evaluated. Reliability was evaluated by correlation coefficient r and Cronbach’s α coefficient; construct validity was analyzed by factor analysis method of appraisal; criterion validity was analyzed by the correlation coefficient analysis with NIHSS scale as the criterion. Sensitivity in various fields was assessed through standardization of effect (SES).Results: Totally 123 valid questionnaires were collected. CSS showed high intrarater reliability, interrater reliability (0.911-1.000) and good internal consistency, with the Cronbach’s α>0.8. There was concurrent validity between CSS and NIHSS (r=0.86). The prognosis prediction accuracy of CSS was 92.4%, slightly lower than that of NIHSS (94.1%). Logistic regression showed that CSS’s “gaze function” and “facial paralysis” were not included in the prediction equation. The facial paralysis had a SES of 0.38, all others had a SES higher than 0.5. Most fields showed a good sensitivity.Conclusion: CSS shows an acceptable reliability, validity and sensitivity in patients with stroke, but the predicative validity of CSS is inferior to that of NIHSS, which needs be further revised.

    Objectlve:To assess the clinical outcome of autologous fat injection in treatment of hemifacial atrophy, so as to search for an ideal treatment of hemifacial atrophy. Methods: Autologous fat granules were obtained by tumescent liposuction. After purification, the fat granules were injected in a multi-strata, multi-tunnel, and multi-spot manner to repair facial introcession abnormality. Patients whose introcession was not improved after the stabilization of the fillings were re-injected with autologous fat granules twice or more until satisfactory outcomes were achieved. Results: All the 7 patients in this study obtained satisfactory outcomes after fat injection and basically reached symmetry. The follow-up result was good after 2 years. Conclusion: Local injection transplantation of autologous fat granules is effective for the treatment of facial depression and hemifacial atrophy, with slight trauma, little complication, and satisfactory outcome.[

    Abstract:
    Obesity is a risk factor for type 2 diabetes, cardiovascular diseases, and various cancers. The prevention and treatment of obesity have become the focus of public health. Omega-3 polyunsaturated fatty acids (n-3 PUFA) are polyunsaturated fatty acids extracted from deep-sea fish. The anti-inflammatory and triglyceride-lowering properties of n-3 PUFA are well known, but its role in the treatment of obesity is still controversial. In animal models, n-3 PUFA can effectively reduce fat. However, in humans, existing studies have shown that n-3 PUFA may not contribute to weight loss, but may slow down weight gain. N-3 PUFA can improve the metabolic changes associated with obesity by regulating intestinal flora, regulating lipid metabolism, suppressing appetite, alleviating adipose tissue inflammation and altering epigenetic mechanisms, thus playing an important role in the prevention and treatment of obesity and the related metabolic diseases.

    Aortic disease(AD) consists of a series of life-threatening diseases caused by aortic wall lesions. Up to now little has been known about the etiology of non-hereditary AD. Vascular smooth muscle cells(VSMC) are the major components of the medial aortic wall and can toggle between a contractile phenotype and a synthetic phenotype. The excessive transformation of VSMC from contractile state to synthetic state plays an important role in the development and progression of AD. Although there are many researches on the regulation of VSMS phenotype switch, how these regulatory pathways operate harmoniously and the what relationship between them remain to be elucidated. Here in this paper we reviewed the existing regulatory mechanisms of VSMC phenotype switch.

    MicroRNAs (miRNAs) play important roles in regulating inflammatory responses, and the related mechanism and regulation characteristics are unique in immune system. In recent years, a large number of studies have showed that miRNAs can target mRNAs of many target genes in Toll-like receptor/nuclear factor-κB (TLR/NF-κB) signaling pathway, and play regulatory roles in immune recognition and inflammatory responses. In this paper we reviewed the characteristics of miRNA in inflammatory reactions, with special attention on the regulatory roles in TLR/NF-κB signaling pathway.

    Regulation between genes is a dynamic event associated with changes of time and circumstances.Gene regulatory network is a complicated and dynamic system.Time series gene microarray provides a tool for creating dynamic gene regulatory network.In this paper,we review several models of dynamic gene regulatory network based on time series gene expression data,including temporal Boolean network,differential equation,dynamic Bayesian networks,etc..The advantages and disadvantages of the models were analyzed and the future of the research is predicted.
