目的 探讨带隐神经足底内侧动脉蒂联合皮瓣修复对侧足底前跖区皮肤软组织缺损的可行性。方法 2007年1月至2009年4月,采用带隐神经足底内侧动脉蒂联合皮瓣修复对侧足底前跖区皮肤软组织缺损12例。交腿皮瓣转移5例,游离皮瓣7例。患者均为男性,年龄6~40岁。成人(11例)联合皮瓣切取范围12 cm×7 cm~13 cm×10 cm,儿童(1例)联合皮瓣切取范围8.5 cm×6 cm,供区游离植皮。交腿皮瓣3~4周断蒂。结果 术后所有皮瓣全部成活, 12例均获随访6~12个月, 皮瓣色泽、质地、外形良好。再造前跖区两点辨别觉6~9 mm。患肢活动自如,皮瓣无溃疡发生。结论 带隐神经足底内侧动脉蒂联合皮瓣可修复对侧足底前跖区较大皮肤软组织缺损,切取面积大,皮瓣质地外观好,可恢复有效两点辨别觉,操作较复杂是其缺点。 |

Objective To explore the feasibility of repairing opposite side ventriplanta soft tissue defects with the medial plantar artery combined flaps with saphenous nerve.Methods From Jan. 2007 to Apr. 2009, 12 patients with opposite side ventriplanta soft tissue defects were repaired using the medial plantar artery combined flaps with saphenous nerve. Five cases used cross leg flaps, and 7 cases used free skin flaps. All the cases were men, with an age range of 640 years (only one child case). The sizes of flaps were 12 cm × 7 cm13 cm×8 cm for adults and 8.5 cm×6 cm for child. The pedicles of cross leg flaps were cut off after 34 weeks.Results All the 12 flaps survived. The patients were followed up for 6 months to 1 year. The color,texture and appearance of the flaps were satisfactory.The flaps of the two point discrimination were 69 mm on ventriplanta of reconstruction, and the affected limb could exercise freely, without ulcer on the flap.Conclusion The medial plantar artery combined flaps with saphenous nerve can be used to repair opposite side ventriplanta vast soft tissue defects, with large flap and good texture, appearance. It can effectively recover the two point discrimination, but it has a complicated protocal. |
