目的 探讨彩色多普勒超声在布氏杆菌性睾丸附睾炎临床诊断中的应用价值。 方法 布氏杆菌性睾丸附睾炎患者35例,年龄24~68岁,平均(44.9±13.0)岁,以阴囊肿痛首次就诊,临床确诊为布氏杆菌病。正常男性志愿者30例作为对照组,年龄21~65岁,平均(43.0±11.6)岁。对两组受检者均行右侧阴囊超声检查,检测指标包括睾丸体积、附睾头尾部厚度、睾丸动脉的血流速度和阻力指数等。对两组数据进行统计分析,比较其差异。 结果 与对照组相比,布氏杆菌性睾丸附睾炎患者睾丸体积增大(P=0.010),附睾头部和尾部增厚(P=0.008,0.007),差异具有统计学意义;睾丸动脉收缩期峰值速度和舒张末期血流速度增快(P=0.000),阻力指数显著降低(P=0.000),差异具有统计学意义。结论 临床表现为阴囊肿痛的患者,结合与病畜及其畜产品接触史,超声检查可以为布氏杆菌性睾丸附睾炎的诊断提供依据,为防止出现严重并发症争取治疗时间。 |

ObjectiveTo explore the value of color Doppler ultrasound in the diagnosis of the brucellosis testicular epididymitis. MethodsThe brucellosis testicular epididymitis group included 35 male patients aged 24-68 years old (\[44.9±13.0\] years), who had scrotal swelling and pain for the first time and were clinically diagnosed as having brucellosi. The control group was composed of 30 healthy male volunteers aged 21-65 years old (\[43.0±11.6\] years). All the subjects received the right scrotal ultrasound examination, including the testis volume, caput and cauda epididymidis thickness, blood flow velocity of testicular artery and resistance index. The data of the two groups were statistically analyzed and compared. ResultsCompared with the control group, the brucellosis testicular epididymitis group had significantly increased testicular volume (P=0.010) and thickened caput and cauda epididymidis (P=0.008,0.007). Moreover, the testicular artery peak systolic velocity and end diastolic velocity in the brucellosis testicular epididymitis group were significantly increased (P=0.000) and the resistance index was significantly decreased (P=0.000). ConclusionFor patients with scrotal swelling and pain, ultrasound examination can provide evidence for diagnosis of brucellosis testicular epididymitis when combined with exposure history to sick animals and livestock, helping to prevent serious complications. |
