目的 采用3.0 T高分辨率磁共振成像(high-resolution magnetic resonance imaging, HR MRI)技术评估大脑中动脉粥样硬化性狭窄,探讨粥样硬化性大脑中动脉狭窄的斑块分布特性及血管重构特点。方法 对64例粥样硬化性大脑中动脉M1段狭窄患者进行3.0 T HR MRI检查,测定斑块厚度并统计斑块部位,计算斑块面积比(斑块面积/斑块所在层面管腔的截面积)、血管重构率(斑块所在层面血管的截面积/近远端正常血管面积的平均值)及残腔率(剩余管腔面积/近远端正常管腔面积的平均值)。结果 大脑中动脉M1段粥样硬化斑块最易形成环形及沉积在管腔前下壁,斑块厚度以0.5~1.5 mm为主(72.5%);大脑中动脉粥样硬化性狭窄血管管腔以正性重构为主。结论 3.0 T HR MRI有利于评估大脑中动脉段粥样硬化性狭窄斑块分布及管壁重构模式,值得进一步深入研究。 |

Objective To apply 3.0 T MRI(3-dimensional time-of-flight and high-resolution MRI vessel wall imaging) technique for analyzing atherosclerotic middle cerebral stenosis, location of the plaque and remodeling of the middle cerebral artery. Methods 3.0 T MRI was used to examine 64 patients with middle cerebral atherosclerotic stenosis, determining the plaque location and thickness. The plaque area ratio was defined as plaque area/cross-sectional area of the vessel(VA); remodeling ratio was defined as VA/average of the proximal and distal normal artery; and the luminal area of the vessel (LA) ratio was defined as LA/average of the proximal and distal normal artery. Results The atherosclerotic plaque of middle cerebral artery (M1 segment) most frequently formed annular plaque and was located at the ventral and inferior wall, with the plaque thickness being 0.5-1.5 mm (72.5%).The middle cerebral atherosclerotic plaque remodeling modes were mainly positive (outward) remodeling. Conclusion 3.0 T MRI can help to demonstrate the distribution of the middle cerebral atherosclerotic plaque and the remodeling of the middle cerebral artery, which is worth further studying. |
