根尖外吸收发生于牙合创伤、矫正治疗、细菌感染或根管系统封闭不完全而导致根尖区域呈弹坑状或火山口样,这可能使牙根尖表面变形而引起根尖封闭的丧失。根据吸收的不同程度,采取不同的治疗方法。一个31岁男性病例,右下侧切牙龈瘘,松动2度,影像学表现根尖外吸收呈喇叭口样、根尖区骨质稀疏影,牙周膜间隙明显增宽。在根管机械、化学清理和根管消毒后,氢氧化钙被输送至根尖区域,厚约3毫米。根管系统充填后,玻璃离子加复合树脂冠向封闭。术后1月、3月、6月、12月跟踪随访,影像学检查显示根尖吸收区修复良好,临床表现为该牙齿功能良好,无松动。治疗结果显示氢氧化钙较成功地用于根尖外的吸收。 |

Calcium hydroxide used as apical barrier in external apical root resorption: a case report |

Abstract: |
External root resorption may occur as a consequence of trauma, orthodontic treatment, bacterial infection or incomplete sealing of the root canal system, and lead to crater formation on the resorbed apex. This would deform the root apex surface, and cause loss of apical constriction. Depending on the extent of the resorptive process, different treatment regimens have been proposed. A 31-year-old male patient, right lateral incision gingival fistula, loose 2 degrees, imaging manifestations of external apical root absorption is blunderbuss, the radiograph of apical bone is very discrete, periodontal membrane space widened obviously. In the root canal machinery, chemical cleaning and disinfection of root canal, calcium hydroxide is conveyed to the apical region about 3 mm thick. After the root canal system was filled, the glass ionomer cements and composite resin was filled crownly. These 1-month、3-month、6-month、12-month radiograph showed that the repair of the root absorption area is good, the clinical manifestations of the tooth function is good, no loose. This case shows that calcium hydroxide is successfully used for external apical root resorption cases. |
